Some PHP help please

I tried installing PHP into my Abyss web server and this is what I get "Notice: Undefined index: ref in Z:\Abyss Web Server\htdocs\phpaffiliate\affiliate.php on line 3" could anyone hlpe me figure out whats wrong? Also, here is the url: http://68.35.164.177:98/phpaffiliate/affiliate.php

Do any scripts work?

Try:

<?
phpinfo();
?>

and name it phpinfo.php, does this script work?

ok, try resetup of php in Abyss, does that solve your problems?
