I have a Westell:Model 2200 router. How/where can I locate the host name and server name???

http://www.aprelium.com/ip/ to get your external IP.

Open a command prompt (Start->Run->CMD) and then type IPconfig to check your machines local IP. The Gateway shown should be your router's IP. You will need to login to your router and setup port forwarding.
