is it possible for a form to have two actions?

what i mean is i have a erl script which signs up new users for web hosting by
default the script only emails me the following

user name
email
real name

so what i need is more info from the form but the script will ignore any info oother than above
so would it be poss to input more fields in the same form and send them to me via e mail as well as run the script ie signup?

you can add the fields into the current form and then open up the perl script and modify it.
