I know how to create the form but how do I get the darn thing to execute and capture the info I want?

Well as Yeshuati posted the actions you need to do depend on how you wan the form proccessed.
Most common proccessing is mailing it through a script. For that you will need a good script to do the trick. Otherwise, you can get the form to writhe the data to a file etc.
