Created and htm file with your script and still just a blank page.
try using the ".php" extension (without the quotes)

Or a .html as this extensions also is included in his php-extensions.

My quess was that all the requiremenst were met. Who wants to install a webmail appliaction without having a mailserver for example ;-)

For those who wonder, yes I do run squirrel mail myself....

Just place all sources in a diretory (eg webmail) in your root directory, create a squirrel directory outside your root directory, edit the configuration file, and you'r ready to go.
