That seems strange. Are these #include directive referring to paths/files that are subject to URL rewriting?

No, not as far as I can see - all my rewrites have Apply to subrequests OFF.

Well OK I hear what you are saying. However these internal server errors on the SSI includes ONLY occur when I turn off the rewrite log - turn logging on again and the errors go away.

It's actually twice as large: 2^32 = 4294967296 seconds (around 136 years: it should be long enough to keep the hackers out of your server until the next technological revolution ;-) ).

What is the maximum value for the anti-hack "Banning Duration"? I'm guessing its a 32 bit signed integer so 2147483647 but I'd like to be sure.

404's are half a bad request, so 2 404's is 1 bad request. Maybe set the limit a little lower? Block after 3 bad requests in 60 seconds? That would probably be better.
